• Accommodating Students with Special Dietary Needs

    The School Nutrition Program accommodates students with special dietary needs if a medical statement form is submitted and the student qualifies with an identified disability, an Individualized Education Program (IEP) or 504 Plan. The medical statement form must be completed and signed by a licensed physician or medical authority.

    If the student’s diet changes during the school year, a new medical statement form must be submitted to the school cafeteria manager, school nurse or mailed to the School Nutrition office. 

    The medical statement form is kept on file at the School Nutrition office.  Menu changes are made through the School Nutrition Director and are communicated to the school cafeteria manager and school nurse (if requested).
