APPLICATIONS MUST CONTAIN ALL REQUIRED ENROLLMENT DOCUMENTS AS STATED ON THE APPLICATION IN ORDER TO BE PROCESSED AND TO RECEIVE A SCHOOL ASSIGNMENT.

**PLEASE NOTE THAT OUT OF DISTRICT APPLICANTS ARE NOT ELIGIBLE FOR TRANSPORTATION TO AND FROM THEIR HOME ADDRESS AND ARE REQUIRED TO PAY THE ANNUAL TUITION AMOUNT AND BE RELEASED FROM THE HOME SCHOOL THAT THEY ARE ZONED FOR.
